通常可加的情況：
(1) 兩邊子句或是片語太長，加逗點分隔開來，看起來叫清楚
(2) 兩邊語意不同，加逗點已示區隔

關於 not only... but (also)... 的用法如下：
1) not only + 受詞O ... but also + 受詞O
因為not only.. but also是屬於對等連接詞所以後面所接的受詞要一致ㄛ。
ｅｘ:
(受詞為名詞) She is not only a well-beloved teacher but (also) a considerate wife.
(受詞為動作) He can not only eat over 7 scoops of icecream but (also) eat 2 bowls of istant noodles.
2) 倒裝句： Not only 助動詞 + S + V, but (also) + V..
not only.. but (also) 如果中間要加逗點的話，比較常見是在倒裝句喔！
ｅｘ： 以 1) 的第二句來改~
Not only does he eat 7 scoops of icecream, but (also) eat 2 bowls of instant noodles.

1) 為什麼第二句沒有主詞he呢?
因為是倒裝句的句型
2) 為什麼eat三單卻沒加s?
因為是受到前面助動詞的影響,等於說是 Not only 助動 s + V..., but (also 助動+S) + V...

基本上, 加逗點或不加逗點都是合乎文法的. 在國外的大學英文課裡,兩種寫法教授都認同. 但是爲了教授的閱讀方便著想, 加了逗號是清楚很多. 當教授能輕鬆的閱讀你的作文時, 分數自然會比較高囉.
