給銀行的英文授權書該如何寫

我有美國銀行開給我的支票,可是過期了.銀行要我到美國辦理可是我沒辦法,所以想寫授權書給在美國的親戚幫忙辦理,不知道該怎麼寫比較適當!

PS. all banks have their own forms. but you will have to show up and

sign the power of attorney form(s) in person, in order to authorize

someone else the banking rights on your behalf; power of attorney is

only good for a period of time, it must be renewed for that person to

continue his/her access to your bank account
