我加拿大bc.省 的care card 不見了

我想要重新辦一張 我疑惑的是 MSP 要先叫我填replacement的form然後寄去那邊的地址(包括full name,birth,birth date,personal health number)但是replacement for the care card 需要20塊

那我要去哪裡附錢啊 那張form上有說 ''do not mail cash''

所以我要先寄那張form嗎? 寄了之後MSP要怎麼通知我去哪付錢啊@@

The cheque for the replacement CareCard should be made payable to the Minister of Finance and mailed to:

Medical Services Plan

PO Box 9667 Stn Prov Govt

Victoria, B.C. V8W 9P6

就是告訴你要寄支票,pay to the "Minister of Finance" ,連同那張form一起寄到
