Androgen Pathway

  In the androgen pathway, cholesterol is initially synthesized to pregnenolone, which undergoes a series of changes ultimately resulting in the synthesis of testosterone(see Figure 1). In CAH, the lack of 21-OH results in elevated levels of androgen precursors (17 OH-progesterone and 17 OH-pregnenolone) that are then converted into the androstenedione and dehydroepiandrosterone. This creates a domino effect, resulting in high levels of testosterone (Sperling, 2002).

Sexual Development

  Genetic sex is determined at conception through the combination of sex chromosomes. Normal female karyotype is 46, XX, and normal male is 46, XY. Prior to the eighth week of fetal development, the external genitalia remain undifferentiated in the embryo (Moore & Persaud, 2003). The differentiation into male and female reproductive structures is a complex series of events. It involves the development of both internal and external structures.

  Internal growth of the male and female reproductive structures rises from two pairs of ducts (Moore & Persaud, 2003; Parker, 1998). The mesonephric ducts, also referred to as the Wolffian ducts, give rise to the male reproductive structures, The paramesonephric ducts, or mullerian ducts, give rise to the female reproductive structures. Testosterone influences the growth of the male reproductive structures from the mesonephric ducts. Mullerian-inhibiting substance, produced in the testes, is essential for the development of the normal male internal genital structures. The presence of this substance causes the paramesonephric ducts to regress, resulting in the formation of the male reproductive structures (Parker, 1998). In the absence of testosterone, the mesonephric ducts will revert, and no further development will occur. Without the testosterone and mullerian-inhibiting substance, the paramesonepric ducts will continue to develop into the female reproductive system.
