not only but also的用法

Question

請問一下not only but also, either or, neither nor.
   這些連接詞如果是用在疑問句的話，前面的助動詞
  或是 be動詞要看哪裡的主詞來決定呢？ 

  例：Not only you but also he likes helping the sick.
   →  ____ you but also he like helping the sick?
        空格要看誰決定助動詞
  還有either or和 neither nor疑問句時，又要看哪個主詞決定呢？
