It has been known for several decades that ICP

cannot be accurately estimated simply by observing

clinical signs such as pupillary size, reactivity or motor

response. Just as optimal control of blood pressure is

facilitated by appropriate blood pressure monitoring, a

serious brain insult requires continuous ICP monitoring.

The main objective of ICP monitoring, therefore, is to

help the physician maintain adequate cerebral perfusion

pressure and oxygenation, and to avoid further medical

and surgical complications.11-13 ICP monitoring has

become an integral part of the management of severe

head injury in virtually all of the foreign research centers

investigating this potentially fatal insult. Some studies

have shown that patients with GCS scores of 8 or less

after resuscitation should have ICP monitoring.14 Patients

with GCS scores of greater than 8 should be considered

for ICP monitoring if they require treatment that would

not allow serial neurological examination,15 such as prolonged

anesthesia for surgery to treat multiple injuries or

prolonged pharmacologic paralysis for ventilatory management;

or if they require a treatment that might raise

ICP, such as positive end-expiratory pressure. Since ICP

monitoring is not funded by National Health Insurance in

Taiwan, its utilization is not yet universal. These three

case reports are presented in order to emphasize the

importance of ICP monitoring for cases of severe head

injury, especially during surgery.
