caretaker can be someone whose job is to look after a house or land while the person who owns it is not there. Janitor is one of caretakers.

caregiver is someone who takes care of a child or sick person. It isn't always free. You still have to pay fees to daycares or nursing homes if you have their services.

In the US, we also call the person who keeps our grass mowed and fence in good condition a caretaker, too.

For those who get paid for taking care of others, they are usually called "care worker". "Caregiver" usually refers to your relatives or friends who support you or your family with disabilities, free of charge.

For those who get paid for taking care of others, people usualy refer them to "care worker". In the US and Canada, caregiver normally refers to your relatives or firends who support you or your family with disabilities (certainly most of the case it is free).
