So, if we have a quality problem due to a faulty printed circuit board (PCB) used in step six, the root cause code would be 1.2.3(A). When the screw is faulty, the code would be 1.2.3(B). Similarly, each incident can be investigated and assigned to one of the unique codes in the process maps.

Table 1 (p. 41) shows a summary of the process map. This gives us useful information about the nature of the production process and the types of components involved.

In a lot of situations, it wasn’t feasible to map all the processes. In these cases, we mapped only the manufacturing processes and not the supporting processes such as purchasing and planning. We also developed a list of other codes to categorize the issues whose root causes were not mapped:

•99.1: material shortage.

•99.2: inventory management.

•99.3: customer related.

•99.4: other.

The severity and nature of the quality problems often vary widely, and it is always better to differentiate the cause by the nature and magnitude of the quality problem. For example, we may want to distinguish between cosmetic and functional quality problems. We added special characters to the process map code to facilitate this. For example, if there is a small scratch in the PCB (Figure 1) it would be coded as 1.2.3(E)-, whereas a functional electrical failure in the PCB would be coded as 1.2.3 (A) without any special characters.

Similarly, an incoming nonconformance in a PCB will be coded as 1.2.3(A)!, whereas a design failure will be coded without any special characters. This technique also forces us to identify the root cause and not the blatant symptoms or effects.
